The Health Risk Coordinator facilitates member outreach for wellness programing in alignment with the overall member engagement strategy and initiatives of the Scott & White Health Plan.  Assist in collecting and analyzing member data for the purpose of developing and implementing innovative wellness programs that include outreach to Health Plan members to encourage Healthy Lifestyle and appropriate use of preventive health services.  Provides leadership, coaching, and overall project support for Institutional Health Risk improvement studies and initiatives of the Scott & White Health Plan.

WHAT IS EXPECTED (ESSENTIAL FUNCTIONS):
Develop and implement innovative wellness programs to ensure progress toward overall objectives is occurring within the planned time frames. Obtain and provide educational materials on the appropriate use of services and on preventive health measuresCollaborate with community-based organizations which may include health promotion fairs or community based organizations for health related initiatives of their programsCollaborate with SWHP QI staff to identify members who may be at special risk and target these groups to urge them to adopt healthy lifestyle and use of appropriate preventive health servicesCoordinate and implement wellness programs that includes outreach to members of employer groups and provide assessments and referrals to appropriate resources within Health Services DepartmentProvide effective educational programs, updates educational programs, objectives and informational materials to reflect current health educational trends Support Health Risk improvement initiatives in order to improve the overall health outcomes of members of SWHP.  Develop and coordinate health risk and wellness initiatives to ensure members at risk or are rising in risk (e.g. Have chronic illness that need to be better managed, on multiple medications and need monitoring) are identified for appropriate interventions and service availableProvide valuable assistance to teams in understanding and effectively utilizing the Health Risk processes and toolsAssist with gathering data and the support of the preparation and submission of regulatory agency surveys to ensure compliance. Assist with the preparation for NCAQ and TDI surveys in order for SWHP to be in full complianceCollect and analyze a wide variety of health care statistics in partnership with various SWHP departments to support health risk/wellness improvement initiatives and projectsDevelop and effectively maintain accurate and reliable methods for collecting, aggregating, tracking and trending data for periodic reviewThe information contained in this job description is intended to describe the essential job functions required of those assigned to this job. KNOWLEDGE, SKILLS AND ABILITIES:
Intermediate level of computer knowledge and of office softwareExcellent communication, organization and prioritization skills are E2695required
Some travel to regional sites will be required

MINIMUM REQUIREMENTS ADDENDUM:
Two years of experience in Health Risk improvement or wellness programming, including data collection, analysis is required
Experience in team facilitation and statistical process control is preferred
Experience in a health related field or activity is preferred
Master's Degree is preferred
